    FA0 field 29, position 140, is the review code indicator. This is an indicator that tells the payer if the claim was under previous review. This is sent for claims that have usually been previously submitted.

    FA0 field 30, position 141, is the multiple procedure indicator. This field, when checked, tells the payer if more than one procedure was performed in relation to the claim being submitted. In some cases, only the first procedure is covered, especially if it is elective surgery.

    FA0 field 31, positions 142 - 151, is the mammography certification number. This is a number that needs to be filled in if the procedure being billed involves a mammography. This is the ID number of the mammography itself for that particular patient.

    FA0 field 32, positions 152 - 160, is the class findings. This field is currently not supported by NSF 3.01 specifications so we'll skip over it and move on to the next field.

    FA0 field 33, positions 161 - 163, is the podiatry service condition. This is a three character code for the procedure if it involved a podiatry service. This field is rarely used.

    FA0 field 34, positions 164 - 178, is the CLIA number. CLIA stands for Clinical Laboratory Improvement Amendments. This program was set up to insure quality lab testing in the diagnosis of various diseases. This field transmits the testing number for this patient if indeed any clinical tests were done.

    FA0 field 35, positions 179 - 185, is the primary insurance amount paid. This is the amount paid by the patient's primary insurance company on this claim. This amount will be filled in if the claim was billed previously and is being re-billed in order to get additional monies.

    FA0 field 36, positions 186 - 187, is the procedure modifier 4. In some cases of medical billing a 4th modifier has to be transmitted, usually for a type of rental equipment during a certain month. This field allows for that 4th modifier to be transmitted.
